Ordering Your Repeat Prescription

If you are on repeat medication, please use the reordering slip attached to your repeat prescription.  Remember to tick the boxes for the items required.

You are also able to either request this electronically via SystmOnline, the NHS App, or simply send an email to: chapel.street93@nhs.net

Alternatively, please put your request in writing giving your full name, date of birth, and the medication required. Disabled patients and the elderly unable to get to the surgery, may telephone their request.

For all prescription requests, please allow 72 hours for the request to be processed and sent to the pharmacy.

Electronic Prescribing

We are now able to use ETP prescriptions, which is electronic prescribing, this new system replaces paper prescriptions.

How it works?

It will not affect you; the process of requesting will stay the same, but instead of paper it will electronically go over to a Pharmacy of your choice. To allocate a nominated pharmacy, please speak to the reception team.

Prescriptions Charges and Exemptions

Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs).

The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.

NHS charges

These charges apply in England only. In Northern Ireland, Scotland and Wales prescriptions are free of charge.

  • Prescription (per item): £9.65

  • 12-month prepayment certificate (PPC): £111.60

  • 3-month PPC: £31.25

If you will have to pay for four or more prescription items in three months, or more than 15 items in 12 months, you may find it cheaper to buy a PPC.
